New name reflects expansion of FUHN’s reach

In June 2025, FUHN changed the “U” in its acronym from “Urban” to “Unified” to better reflect the clinics that now comprise the network.
When 10 federally qualified health clinics (FQHCs) formed the Federally Qualified Health Center Urban Health Network in 2013, they were all based in the urban centers of Minneapolis and St. Paul. From the start, the primary goal was to pursue an accountable care organization (ACO) through a Medicaid demonstration project with the state of Minnesota. FUHN
adopted an ACO model, taking responsibility for the total cost and quality of care for a defined patient population and integrating primary care with expanded technology to enhance care for Medical Assistance patients.
Over the past several years, FUHN has expanded its membership with two FQHCs: Community Health Service, Inc. (CHSI) and Open Door Health Center (ODHC). The addition of CHSI and ODHC expanded FUHN’s reach to include broader reach in Minnesota and North Dakota. CHSI is a network of community clinics in Minnesota and North Dakota, with clinics in Moorhead, Rochester, Willmar, and Grafton, along with a victim advocacy services office in Crookston. ODHC has a medical, dental, and behavioral health clinic in Mankato and a dental clinic in Jordan. ODHC will also be part of the new Scott County Integrated Health Center in 2026 and is opening two school-based clinics in partnership with Mankato Public Schools this fall, in addition to the school-based clinic it opened in Fairmont in 2024.
