FUHN: Helping to improve healthcare in community clinics

The Federally Qualified Health Center Urban Health Network (FUHN) is a 501(c)3 nonprofit of 11 federally qualified health clinics (FQHC), predominantly based in the Twin Cities and Mankato metro areas. FQHCs are community-based organizations that provide comprehensive primary care and preventive care, including medical, dental, and mental health/substance abuse services to people of all ages and backgrounds, regardless of their ability to pay or health insurance status. FQHCs are a critical component of the U.S. healthcare safety net.

In 2012, 10 founding clinics formed an Integrated Health Partnership (IHP) in partnership with the Minnesota Department of Human Services on Medicaid healthcare reform efforts. Through the initial arrangement within the IHP, the participating clinics achieved more than $26 million in savings for the state. Today, these clinics aim to further enhance the care delivered to Medical Assistance patients by coupling primary care with expanded use of technology and improved outreach.

FUHN also serves as a Health Center Controlled Network (HCCN), which is funded by the Health Resources & Services Administration, to strengthen health information technology (IT) to improve operational and clinical practices that result in better health outcomes. FUHN supports its member clinics in leveraging health IT and data to deliver high-quality, culturally competent, equitable, and comprehensive primary health care and effectively participate in value-based care arrangements.

 

The FUHN team has a focus on improvements in clinical quality, patient-centered care, and provider and staff well-being. Together FUHN and the participating clinics have a shared vision for delivering high-quality healthcare for underserved populations in the Twin Cities and surrounding communities. FUHN has a strong and active board, and its committees and work groups collaborate on shared objectives, from facilitating cost-effective procurement processes to performing data analytics to support quality initiatives.
