Swetha Pradeep, PharmD

Swetha practices comprehensive medication management at several of the FUHN clinics and leads a team of pharmacists across the network who are implementing this patient-centered service. She completed her doctor of pharmacy degree at the University of Minnesota and her residency training at Community University Health Care Center (CUHCC). Her view on patient care is holistic, understanding that many external factors contribute to a patient’s health and that a patient can benefit from the combined expertise of an interprofessional team, including a pharmacist.
