RESULTS

Federal Urban Health Network has achieved amazing results over the life of its IHP contract. 

Integrated Health Partnerships (IHPs), or Accountable Care Organizations (ACOs) are provider and payer arrangements established to improve care coordination between primary care physicians, hospitals, specialists, and public or private health payers.

The IHP between Minnesota Department of Human Services and FUHN has resulted in reduced ED utilization, reduced hospital inpatient admissions, and a total savings of over $26,000,000 since 2012. 

FUHN and its member clinics are committed to Healthcare's triple aim.
